Return Quotient · The Missing Intelligence
IQ measures your intellectual intelligence.
EQ measures your emotional intelligence.
RQ measures your restorative intelligence.
The missing intelligence.
You have optimised everything medicine offered. Something is still wrong that nobody has named — or prescribed for. RQ is the framework that addresses what was missed.

You May Recognise Yourself Here
After thousands of consultations, I identified two profiles. Apparently different. The same underneath.
Profile A
Optimised on paper.
Dying inside.
- Blood work normal. Biomarkers optimal.
- Sleep tracked. Diet controlled. Steps counted.
- No clinical diagnosis. No obvious reason.
- A profound emptiness you cannot name.
- Something is wrong that medicine has no code for.
Profile B
Unwell. And failed
by the system.
- Chronic illness. Persistent symptoms.
- Diagnosed, labelled, medicated correctly.
- The system did everything it was designed to do.
- And yet the root was never reached.
- You return again and again. Nothing changes.
They are not broken. They are unable to return. And medicine has no prescription for the return.
What Medicine Missed
We have built the most sophisticated medical system in history. Best diagnostics. Best drugs. Longest-lived. And the most lost.
Beneath every presentation — the high achiever losing herself, the patient failed by the system, the person who has tried everything and still cannot find the return — was the same thing. Not a diagnosis. A root fear. One of a small number of primal fears that medicine never asks about and never prescribes for.
"The soul is not spiritual decoration. It is the return mechanism. And I am a doctor who prescribes it."— Dr. Safia Debar
These fears live in the body. They activate the nervous system, dysregulate the HPA axis, and produce every chronic presentation I see in clinic. Root fears are biological. Medicine just forgot to prescribe for them.
The Root Fears
One of these is holding your pendulum. Recognising it is the beginning of the prescription.
Not Enough
Worthlessness, inadequacy, the chronic sense of falling short. Most pervasive in high-achieving populations.
Failure
The fear of being wrong or defective. Drives perfectionism, overwork, and the inability to rest.
Abandonment
Not belonging, rejection. The social nervous system under chronic threat. Drives people-pleasing.
Death & Suffering
Loss, pain, the unknown. Often the hidden driver beneath health anxiety and chronic illness.
Loss of Control
The most common of all. The body stopped trusting the return. It grips harder instead of letting go.
Most PrevalentMeaninglessness
The undiagnosed fear beneath the optimised life. No ICD code for existential emptiness.
RQ doesn't just name the fear. It measures it — and provides the clinical prescription to address it.

The Missing Intelligence: A Clinical Framework for Restorative Intelligence and the Measure of Soul Health
RQ White Paper · First Edition · 2025 · Dr. Safia Debar
This white paper introduces RQ as a novel clinical framework grounded in the ACE Study, Polyvagal Theory, and affective neuroscience. It is the first public statement of the RQ framework. Free to read, share, and cite.

"The pendulum was always designed to return. Something stopped it. RQ is the measure of whether it can — and a prescription for restoring the return."
