After thousands of consultations, I identified two profiles. Apparently different. The same underneath.

Optimised on paper.

-Sleep tracked. Diet controlled. Steps counted.
-No clinical diagnosis. No obvious reason.
-A profound emptiness you cannot name.
-Something is wrong that medicine has no code for.

Unwell. And failed
by the system.

- Diagnosed, labelled, medicated correctly.
- The system did everything it was designed to do.
- And yet the root was never reached.

What Medicine Missed

We have built the most sophisticated medical system in history. Best diagnostics. Best drugs.
Longest-lived. And the most lost.

Beneath every presentation — the high achiever losing herself, the patient failed by the system, the person who has tried everything and still cannot find the return — was the same thing. Not a diagnosis. A root fear. One of a small number of primal fears that medicine never asks about and never prescribes for.

These fears live in the body. They activate the nervous system, dysregulate the HPA axis, and produce every chronic presentation I see in clinic. Root fears are biological. Medicine just forgot to prescribe for them.

The Root Fears

One of these is holding your pendulum. Recognising it is the beginning of the prescription.

Not Enough

Worthlessness, inadequacy, the chronic sense of falling short. Most pervasive in high-achieving populations.

Failure

The fear of being wrong or defective. Drives perfectionism, overwork, and the inability to rest.

Abandonment

Not belonging, rejection. The social nervous system under chronic threat. Drives people-pleasing.

Death & Suffering

Loss, pain, the unknown. Often the hidden driver beneath health anxiety and chronic illness.

Loss of Control

The most common of all. The body stopped trusting the return. It grips harder instead of letting go.

MOST PREVALENT

Meaninglessness

The undiagnosed fear beneath the optimised life. No ICD code for existential emptiness.

RQ doesn't just name the fear. It measures it — and provides the clinical prescription to address it.

Awareness

 The capacity to feel what your body is signalling before symptoms appear.
You cannot return to somewhere you cannot feel.

BREATH

CONNECTION

 Connection to self, others, and meaning. Disconnection at any of these three levels holds the pendulum.

 The denominator — the root fear that divides and diminishes return regardless of how strong A, B and C are. The most clinically significant variable.
